Financial Services

ALPHA Behavioral Health Center serves all individuals regardless of ability to pay. ALPHA has adopted the federally developed definition of poverty as the criteria to define indigence for fee determinations. This definition is based on person served/family income and family size and revised annually. If concerned about ability to pay for services, please call our office to find out the qualifications for the Sliding Scale Fee Program or other assistance programs available.

Methods of payment

  • We accept Medicaid and most health insurance plans
  • We accept cash, checks, and most credit/debit cards
  • Individuals may qualify for our Sliding Scale Fee Program after a Financial Assessment is completed with a counselor. Qualifications will be based upon proofs of income and the most recent Federal Poverty Guidelines

For your first visit you need to bring

  • Photo ID
  • Social Security Card
  • Official printout of current medications
  • Any referral information
  • Insurance or Medicaid card

TREATMENT

Confidentiality & CARF Accredidation

All services are considered confidential and are protected by federal law (42 CFR Part 2) and the Health Insurance Portability Act (HIPAA) of 1996.  Due to Confidentiality regulations, all individuals must sign a release of information regarding their protected health status.  Clients and parents of clients under age 16 will also be asked to sign an acknowledgement that says they have received the privacy notice.
ALPHA Behavioral Health Center is a CARF Accredited institution.
